Job Title: Medical Scheduler Case Manager Assistant for Transition of Care
Location: San Francisco, CA
Job Description: LHH Recruitment Solutions is seeking a dedicated Medical Scheduler Case Manager Assistant for an established healthcare client in San Francisco, CA. This onsite role ensures seamless transitions for ambulatory patients from hospital discharge to post-acute care settings. The position is full-time, 8 hours a day from 08:00 to 17:00, for a duration of 26 weeks with the possibility to extend. Key Responsibilities:
Collaborate with Case Managers and Social Workers to develop and implement Transition of Care (TOC) plans.
Communicate with patients and families about post-acute care options and discharge plans.
Monitor and document progress towards TOC goals, addressing any barriers.
Coordinate post-discharge referrals and authorizations.
Facilitate patient transfers to post-acute facilities, including preparing documents and arranging transportation.
Participate in daily team huddles and department meetings.
Maintain accurate documentation in the electronic medical record system.
Provide clerical support as needed.
Qualifications:
High School Diploma or equivalent education/experience.
Minimum of 1-year recent relevant experience.
Strong communication, interpersonal, and time management skills.
Ability to work effectively in a fast-paced environment with shifting priorities.
Proficiency in PC and word processing skills.
