As a Case Manager, you will play a vital role in providing essential support to patients facing various serious mental health challenges.
Your responsibilities will include:
• Meeting with patients on a one-on-one basis to offer compassionate support and guidance.
• Facilitating group sessions to foster a supportive community environment and encourage peer interaction.
• Responding to crisis calls through Blue Dot support to provide immediate assistance and intervention during critical situations.
• Offering resources and referrals to individuals struggling with mental health issues, empowering them to access the help they need.
Qualifications:
• High school diploma required.
• Previous experience in mental health support for adults or counseling
• A minimum of 2 years experience as a case manager working with individuals with severe mental health illness or related field.
• Strong interpersonal skills and ability to empathize with individuals experiencing mental health challenges.
• Excellent communication skills, both verbal and written.
• Ability to handle crisis situations with composure and professionalism.
• Experience with Nextgen software is a plus.
